I understand that The OA Firearms & Survival Training LLC provides State CCW Application Assistance as an administrative support service only. This service may include help organizing documents, reviewing forms, completing application materials, and navigating online submission portals. I understand that this service does not constitute legal advice or legal representation, that no attorney–client relationship is created, and that I have been advised to consult my own attorney for any legal questions about eligibility, disqualifiers, or use‑of‑force law.

No Guarantee of Outcome:

I acknowledge that The OA Firearms & Survival Training LLC does not guarantee approval, renewal, issuance, or processing time for any concealed carry permit application. I understand that all final decisions are made solely by the applicable government agency or licensing authority.

Accuracy Responsibility:

I understand that I am solely responsible for the accuracy, completeness, and truthfulness of all information and documents submitted in connection with my application. I agree to review all forms and materials before submission and accept responsibility for any errors, omissions, missed deadlines, or failures to comply with applicable laws or agency requirements.

Initial Release and Indemnification:

To the fullest extent permitted by law, I hereby release, waive, and hold harmless The OA Firearms & Survival Training LLC, its owners, officers, employees, contractors, agents, instructors, affiliates, and representatives from any and all claims, liabilities, damages, losses, costs, or expenses arising out of or related to the State CCW Application Assistance services provided to me, including but not limited to application delays, denials, requests for additional information, revocations, or other adverse outcomes.

Hold Harmless and Indemnification:

I further agree to indemnify and hold harmless The OA Firearms & Survival Training LLC from any claims or demands arising out of my application, my submitted materials, my representations to any agency, or my failure to comply with applicable laws, rules, or procedures. I acknowledge that I have read and understood this waiver, that I have had the opportunity to ask questions, and that I sign it voluntarily.
